Webinar Highlights: Lessons from Six Years of AI-Enabled Stroke Workflows

What happens when AI has been embedded into a stroke workflow for six years? In a recent webinar with Health AI Register, Dr. Martijn Boomsma, Radiologist at Isala Hospital in the Netherlands, shared practical lessons from his team’s experience implementing Aidoc’s AI in daily clinical practice.

Dr. Boomsma explained that the biggest impact has been on workflows, communication and giving radiologists greater confidence in time-critical situations.

“I’m not reporting anymore before I see the AI result, and that’s for a good reason.”

At Isala, AI helps prioritise urgent cases, supports communication across the regional stroke network, and enables clinicians to prepare for thrombectomy before the formal referral call has even been made. The platform also gives on-call teams secure mobile access to imaging, helping specialists collaborate more effectively across departments.

One of the biggest changes has been overnight reporting where there used to be frequent phone calls between emergency physicians and the on-call radiologist. But since Aidoc’s AI was integrated into radiology and emergency department workflows, emergency physicians can manage many examinations without suspected acute findings, while scans flagged as positive are escalated immediately to the on-call radiologist. 

“Sometimes I can sleep three or four hours extra during a night shift.”

Reflecting on six years of experience, Dr. Boomsma believes AI has become an important part of delivering high-quality stroke care. In his view, radiology teams that choose not to adopt AI should recognise they may be missing opportunities to improve patient care and workflow efficiency.

“If you don’t use the AI tools we’ve shown today, you have to accept you’ll perform differently as a radiology group than you would with them.”

At the same time, he emphasised that successful AI implementation depends on much more than technology. Long-term adoption requires clinical leadership, governance, monitoring and continuous user engagement.

“It’s never a technical problem or an ICT problem. It’s all about governance, motivation and feeling the urgency.”

His conclusion was that AI should support radiologists, not replace them. Human oversight remains essential, but when thoughtfully implemented, AI can improve workflows, enhance collaboration and help clinical teams deliver faster stroke care.
